The purpose of this research project is to develop an evidence-based palliative care
program specific to heart failure patients, assisting in the prevention of heart failure
readmissions. The guidelines established by the Center to Advance Palliative Care (CAPC) will
be used to structure the program.
The population of this study will consist of patients with a documented history of heart
failure. A survey of hospitalized patients with heart failure history who were admitted with the
diagnosis of heart failure will be conducted using the Minnesota Living with Heart Failure
Questionnaire® (MLHFQ) to determine the patients’ health-related quality of life. Based on the

This study presents a meta-analysis of current research as to the customer's relationship between magazine's websites and the print product. It will review more than 15 relevant studies to determine if a consensus exists on the degree to which online publishing has hurt print circulation and reader loyalty. The early Internet brought a lot of fear that print magazines, as we know them, would quickly cease to be. While newspapers struggle to remain relevant with Web 2.0, magazines have shown more resilience. The data shows a much slower decline in sales compared to newspapers, but it is to be noted that the decline, while slower, is still measurable. Orthophosphate removal from wastewater in a subsurface-flow constructed wetland (SFCW) was studied during the fall of 1995. The SFCW consisted of a cell with an impervious liner, filled with 1.27 to 3.81 cm river rock and planted with common reeds (Phragmites australis) and soft-stem bulrushes (Scirpus validus). Municipal wastewater was pumped through the wetland and comparisons were made between inlet and outlet orthophosphate concentrations. Comparisons were also made at locations within the cell. A mean orthophosphate reduction of 62% with a range of 91% to 32% was observed for the duration of the study. An ANOVA test showed a significant reduction of orthophosphate in the wetland cell during the study. This research paper looks at the ability of neighborhood organizations to attract businesses to their neighborhoods, and provides recommendations for success. This study concentrated on neighborhoods in Indianapolis, IN. First, the average change in the number of businesses from 2008-2013 for census tracts in each neighborhood was calculated. A survey was then conducted of several organizations throughout the city to determine current actions and effectiveness. Four organizations were then chosen for case studies. The organization leaders were interviewed, as well as some local business owners from the neighborhoods. Each of the organizations had a different role in the attraction of businesses but they were all successful. The spatial relationship between brownfields and low-income and non-Caucasian neighborhoods in the city of Indianapolis was investigated. The primary goal of this research was to determine if brownfields are disproportionately located in these types of neighborhoods. A. Pearson product moment correlation was used to determine whether significant correlations exist between distance and minority percentage and household income. The research determined that no significant correlation exists for these variables. No support for environmental discrimination was found by this study, therefore the evidence suggests that Indianapolis doesn't need to aggressively target these sites for redevelopment based on environmental justice issues.
