Disseminated cryptococcosis mainly occurs in patients with impaired
cell mediated immunity. We present a case of disseminated
cryptococcosis in a non-HIV patient with nephrotic syndrome who never
received immunosuppression. Cultures of bone marrow aspirate,
cerebrospinal fluid analysis and histology of skin lesions were all
consistent with Cryptococcus neoformans infection. Treatment
with amphotericin B followed by fluconazole was successful and in the
course of two months when, the skin nodules disappeared.
The acute schistosomiasis is the toxemic disease that follow the
Schistosoma cercariae active penetration trough screen in the
immunologicaly naive vertebrate host.The clinical picture starts two to
eight weeks after the first contact with the contaminated water.
Susceptible patients present a syndrome comprising fever, diarrhea,
toxemia and hepatosplenomegaly. Diagnosis is based on epidemiological
and clinical features, presence of Schistosoma eggs in the feces,
enlargement of abdominal lymph nodes by ultrasonography and by
detection of high antibodies levels against the antigen keyhole limpet
haemocyanin. Different rates of cure have been observed with specific
medication and for the most severe clinical presentations the use of
steroids reduces the systemic and allergic manifestations.
Aim: We study the electrocardiographic abnormalities at time of
diagnosis of acute PE in our series of consecutive patients of the last
years. We conclude that the electrocardiogram may have diagnostic and
prognostic value in patients with acute PE. Methods: We
retrospectively reviewed the records of 154 consecutive patients with
acute PE.Diagnosis of PE was established by a high-probability
ventilation/perfusion lung scan (121 patients), pulmonary angiography
or spiral computed tomography (19 patients), or the combination of a
suggestive clinical picture with a deep vein thrombosis (DVT)
demonstrated by phlebography or echo-doppler (14 patients). Results:

This paper presents epidemiological, laboratory, and clinical data on
12 years of dengue virus activity in the State of Rio de Janeiro from
the time the disease was first confirmed virologically in April 1986
through April 1998. DEN-1 and DEN-2 viruses are the serotypes
circulating in the state and were responsible for the epidemics
reported during the last 12 years. The results published here show both
the impact of dengue virus infections on the population and laboratory
advances that have improved dengue diagnosis.
The effect of solar radiation, rainfall and cannibalism on the disappearance of eggs and larvae of the cereal stemborer Chilo partellus (Swinhoe) were studied. Disappearance and nonviability of egg batches was higher on plants exposed to solar radiation than on shaded plants. Significantly fewer small larvae were recovered from plants exposed to rainfall than from shielded plants. Cannibalism among larvae of the same size was not common except at high larval densities. Contrary to the common belief that disappearance is the result of predation, these studies show that solar radiation and rainfall might also play an important role in the disappearance of stemborer eggs and small larvae. Gastroepiploic aneurysms are extremely rare. They occur mainly in
elderly men and in 90% of cases are ruptured at presentation. Visceral
aneurysms though rare should be borne in mind in cases of unexplained
haemorrhagic shock. We present a case of a 79-year-old man who
presented with abdominal pain, hypotension and anaemia but no obvious
source of bleeding. He had undergone a prior aorto-bifemoral graft. The
patient refused an operation and died the following day.
Giant vertebrobasilar (VB) junction aneurysms are uncommon aneurysms,
especially those associated with multiple aneurysms of the posterior
circulation. We report two cases, one with a small and a giant aneurysm
of the VB junction which were surgically clipped; and the other with a
small left anterior inferior cerebellar artery (AICA) aneurysm which
resolved spontaneously. The patient, however, developed a de-novo giant
VB junction aneurysm, which was detected on a follow-up angiogram. This
aneurysm was treated by surgical clipping. The clinical features,
angiographic considerations and surgical treatment of such rare
conditions are discussed and the relevant literature reviewed.

Hematogenous spread from carcinoma of tonsil is an uncommon event and
skin is an extremely rare site of metastasis. We encountered a 40 year
old male patient who initially presented with carcinoma of the tonsil
with T3N2cMO disease and treated by curative radiotherapy. After about
2 years, he developed a skin lesion in the periorbital region which on
cytological examination turned out to be metastasis from tonsillar
carcinoma. The present paper describes this rare case report along with
a brief review of the literature.
